What Hand in Hand Home Care can offer your loved one:
Case Management Services
All clients have a case manager assigned to them. The case manager will coordinate all services provided to keep everything organized and as simple as possible. Many of our clients' and their family's physicians, attorneys and private social workers find this service extremely helpful in reducing the number of calls they receive regarding the elder's care. It is also beneficial to a client whose family lives out of state and cannot be involved with doctor's appointments, shopping, and other everyday needs.
Services Performed
Coordinate appointments, confirming staff schedules, follow up appointments with nurse
Ensure weekly food shopping is done
Coordinate all household needs, maintenance etc.
Meet with other professionals such as physicians, lawyers, social workers or guardians to update condition of client
Provide one central person to communicate with all persons involved in the care and comfort of the client

their particular area of service. We screen all of our staff members using the most secure methods available to ensure quality of care and service to provide you with:

Home Care Explained
Home care encompasses a multitude of medical and personal services provided at home to a partially or fully dependent person. These services often make it possible for an individual to remain at home rather than enter a residential facility for extended recovery or long-term care.
Healthcare -- physical and other rehabilitative therapy, medication monitoring
Personal Care -- assistance with personal hygiene, dressing, getting in and out of a bed or chair, bathing and exercise
Nutrition -- meal planning, cooking, meal delivery or meals at outside meal sites
Homemaking -- housekeeping, shopping, home repair service, household paperwork, and
Social and Safety Needs -- escort and reasonable local transportation services ie; local physician or grocery shopping, assistance with location of adaptive equipment, companions, telephone check, overall planning and program coordination service.

with you, we place only caregivers who possess the following attributes: work
experience, professionalism, integrity and good moral character. These
qualifications and attributes are verified through background checks,
previous work references, as well as through continuous process of review
encompassing client feedback and recommendations. This selection process is
designed to provide you with exceptional care.
